AIDS Project New Haven
AIDS Project New Haven provides education, non-judgmental comprehensive and holistic services to individuals infected and affected by HIV/AIDS. Their goals are to increase public awareness, promote prevention through educational programs and advocacy, and secure funding for the support services they provide for their clients.

The APT Foundation
Headquartered in New Haven, The APT Foundation provides treatment to individuals with substance use problems with an emphasis on traditionally underserved populations. Specialized programs are offered for adolescents, addicted women with children, and those living with HIV/AIDS. Services include primary healthcare evaluation and treatment, outpatient counseling, outpatient detoxification, methadone maintenance, residential treatment and vocational services.

Birmingham Group Health Services
The HOPWA program provides housing assistance and support to HIV/AIDS clients living in the Lower Naugatuck Valley area. Services offered include: short-term mortgage, rent and utility assistance (STMRU) accessible for up to 21 weeks in any given calendar year; transitional housing support for up to 12 months and other housing related services such as advocacy, referrals, information, and linkages with existing housing resources. Located in New Haven, Columbus House offers shelter, transitional and permanent supportive housing. In addition the agency provides assessments, case management and programs and services that are designed to address the most common root causes of homelessness including: substance abuse; physical or mental illness; lack of education and job skills; and a person's history of abuse and neglect. CHC of Meriden offers services including: comprehensive medical services, mental health services, dental care, pharmacy services, HIV counseling and testing, WIC health screening, case management, nutritional services, prenatal care, podiatry, laboratory service, Ryan White III early intervention services, HIV primary care, and HIV specialty care.

Independence Northwest
Independence Northwest operates the Transitional Housing Opportunities Program, a ten-unit scattered site program serving the upper Naugatuck Valley and Greater Waterbury regions. The program offers advocacy, case management, emergency assistance funds, independent living skills training, peer counseling, assistive technology services, and referrals. MORE INFO

Leeway
Leeway, Inc. is a forty bed skilled nursing facility located in New Haven. Twenty-four hour individualized nursing care and supervision are provided. Services include physical therapy, occupational therapy, speech therapy, whirlpool treatments, and supervision with medications and treatments. New Haven Home Recovery
NHHR operates an eight-unit apartment building and a 35-unit scattered site apartment program for families with HIV/AIDS in Greater New Haven. In addition, NHHR also operates two homeless shelters for single women or women with children and three non-HIV supportive housing programs. New Opportunities, Inc. operates a seven unit scattered site housing program, and an emergency shelter with HIV/AIDS services and transitional living for the homeless in Meriden. Both programs offer case management services to program participants. MORE INFO
